Problem
BB-PROVISION-01 (historical): Church provisioning failed when registration reused an existing user by phone but submitted a different email.
Root cause
After phone-matched identity reuse, assignBlessBoardRole re-resolved the administrator by unmatched registration email (user_not_found), historically mislabeled database_conflict.
Fix
Canonical administratorUserId for role assign; no email fallback when userId set; mapRoleAssignmentFailureStatus; idempotent assign + rollback/retry preserved
